Unlike edible fungi or mushrooms that live on dead vegetable matter, the fungi and yeast that infect the feet are specialized dermatophytes, meaning that they only feed on keratinized tissue such as hair, skin and nails. Achilles Tendonitis Causes and Treatment
Achilles tendonitis is a common injury in runners and other athletes who run often during sports. The Achilles tendon is the largest and most vulnerable tendon in the body. It joins the gastrocnemius (calf) and the soleus muscles of the lower leg to heel of the foot. The gastrocnemius muscle crosses the knee, the ankle, and the subtalar joints and can create stress and tension in the Achilles tendon. Tendons are strong, but not very flexible so they can only so far before they get inflammed and tear or rupture.
Achilles Tendonitis SymptomsAchilles tendonitis can come on gradually or suddenly. Acute symptoms are hard to ignore and includes pain in teh back of the ankle and just above the heel that comes on quickly over a few days. It increases during exercise (particularly running). Stop speed training and hill running, and begin gentle calf stretching after exercise when the muscle and tendon is still warm and flexible. Post-exercise ice may also help. Be carful to avoid excessive stretching that could aggravate the problem. Strengthening the calf muscle can help reduce the stress on the Achilles tendon. Toe raises, and balancing on your toes, and wall stretching are useful exercises.
It is not necessary to stop activity completely (you may consider cross training) as long as you pay attention to muscle soreness and reduce activity accordingly.

Tens of thousands of patients are being passed around hospitals "like parcels" in the middle of the night to relieve pressure on NHS beds, The Times has learnt. Doctors and patients' leaders attacked the "dehumanising" practice of repeatedly moving vulnerable people when they should be asleep, which leads to longer hospital stays and puts health at risk. Ministers promised yesterday to crack down on needlessly shunting patients. "Appalling" figures show that the number of patients being moved out-of-hours has risen by nearly 20 per cent in five years. One was moved 24 times at night during a long hospital stay. "The biggest concern is not just the stress to the patient but the safety aspect as well," Dr Mark Temple, Acute Care Fellow at the Royal College of Physicians, said. "Continuity of care is central to quality of care and it's absolutely critical in terms of safety." Freedom of Information requests were used to ask every hospital in England for the number of patients who moved beds or wards between 11pm and 6am. In the 58 trusts that gave figures, 195,372 people were shown to have been moved at night last year. Many raised questions over the accuracy of their own data, and others admitted that they did not keep such records. Achilles tendonitis is inflammation of the Achilles tendon found at the back of the heel and ankle. The Achilles tendon is the strongest and largest tendon in the body, running down the back of the lower leg to join the calf muscles to the heel bone. It helps the calf muscles lift the heel off the ground and so is needed during walking and running.

Risks and Causes of Achilles Tendonitis
Those who play sports that involve sudden changes of direction, stops and starts are at risk of developing Achilles tendonitis. These sports include tennis, squash, netball, football and basketball. Uneven ground, incorrect or ill-fitting footwear and inadequate preparation and training add to this risk. A single incident can cause the irritation or, more commonly, consistent stressful movement. Injuries more often occur after a period of inactivity when the tendon is stiff and inflexible.

It isn't only sports players who are at risk of developing Achilles tendonitis. Women who frequently wear high heeled shoes are also at risk because the height of the heel actually has the effect of shortening the Achilles tendon.

This means that when she wears flatter-heeled shoes, the tendon has to over-stretch and tears can occur, which causes inflammation and pain. People who don't exercise often and then play a high-risk sport or suddenly start intense exercise are also at risk of Achilles damage.

Some people have a congenital condition of a short Achilles tendon which makes them more prone to Achilles tendonitis than the general population.

Symptoms of Achilles Tendonitis

Tendonitis symptoms often develop gradually with only moderate pain at first. If left untreated and the activity which caused the pain is continued, more serious damage can occur. You might experience one or more of the following symptoms:

The best treatment options will be determined by the severity of the injury. In many cases, simple treatment can be carried out at home with only more severe or long-standing cases needing specialist treatment. If pain persists after a few days of conservative treatment, seek medical advice to avoid a more serious injury.
Rest is the most important treatment for tendonitis. Continued activity using the inflamed tendon can lead to a more serious problem.Apply the R.I.C.E. Treatment - rest, isolation, compression, elevation. Sit or lie down; take the weight off your foot; apply a compression bandage for the swelling and to limit movement; raise your foot on a pillow so that it is higher than your heart.NSAIDs or anti-inflammatory medications help to reduce the inflammation, pain and swelling. Over-the-counter medicines like ibuprofen or aspirin are effective but your doctor may prescribe stronger drugs if it is warranted.Rest for 24 to 48 hours and then start to gently move your ankle in circles and flexing your foot up and down to stretch the Achilles tendon; this will help get you back on your feet sooner. Resting for too long can be counter-productive as the muscles, joints and connective tissue tighten and it can take you longer to recover.
Other treatment methods that may be needed for more severe or recurring cases include wearing a brace or a cast, with surgery to repair a damaged tendon the last resort. These treatments will mean a longer recovery with extensive rehabilitation needed.

Prevention of Achilles Tendonitis

Achilles tendonitis can be prevented by ensuring a complete warm-up and stretching program is followed before any strenuous sport or exercise is started. If you have had Achilles tendonitis before, pay particular attention to stretching the heel, ankle and calf muscles. Warm up the feet and ankles with light jogging on the spot, lunges and small spot jumping.

Wearing the correct shoes for the activity you are engaged in is vital to preventing Achilles tendonitis. Go to a specialist sports shoe store to be correctly fitted for the right shoes for your sport and for your individual feet. Even if you are just into walking for exercise and fitness, the correct shoes are important to avoid injury which will interfere with your activity. Achilles tendonitis shoes need to have a cushioned heel, a heel cup and good heel support.

If you know that you are prone to tendonitis, avoid pushing yourself. If you experience any pain, stop and apply the RICE treatment. Focus your training and preparation on strengthening your ankles and having a flexible Achilles tendon.

If you love to wear high heels, take a few minutes before you put them on, and throughout the day, to gently stretch your Achilles tendon by flexing your foot up and down and drawing imaginary large circles in the air with your toes.

Always apply the tape on clean feet and keep your feet dry. The effectiveness of taping is based on level of plantar fasciitis In acute cases, plantar fasciitis taping only may not be sufficient to decrease the pain and should be used in addition to arch support or over-the-counter nonsteroidal anti-inflammatory drugs (NSAIDs) such as ibuprofen and naproxen. However, pain relievers do not cure the real cause. That is why plantar fasciitis taping is most effective when combined with treatments techniques that are not meant for symptom relief only but deal with the root reason of the pain as well.

In plantar fasciitis treatment, walking may cause overstretching of your leg. When you walk, walk gently without straining your heels. Another thing to remember is to avoid walking on hard surfaces and always opt for proper well cushioned shoes. Old and worn out shoes must be avoided which can cause discomfort while walking. There are heel pads and cushions available which help provide support to the heels and feet. There are painkillers and other anti-inflammatory drugs that relieve the throbbing pain. In addition, there are certain plantar fasciitis exercises like stretching exercises. However, these exercises must be performed in supervision of a physiotherapist.

Your next Plantar Fasciitis exercise is stretching of the plantar fascia using a bath towel. Put a rolled up towel under the ball of one foot, holding both ends of the towel with your left and right hand. Next, slowly pull the towel towards you, while keeping your knee straight (the other knee may be bent). Hold this position for 15 to 20 seconds. Repeat 4 times and change to the other foot, if necessary. (It's always good to do these exercises on both feet, even if you only experience heel pain in one foot, as this will help prevent the heel problem to come back in your other foot!)

As we stand and apply our weight to the foot, the arch drops and the plantar fascia becomes tightened. Plantar fasciitis occurs when the weight that is applied to the foot is so great that the tension in the plantar fascia increase, causing damage as it begins to pull away from the heel bone. This is a very important concept to understand and is probably why plantar fasciitis is such a misunderstood medical condition. The painful symptoms of PF do not result from standing on the heel, but rather result from overwhelming tension or repetitive stress that is exerted on the plantar fascia as we stand or exercise.

Surgery carries the risk of nerve injury, infection, rupture of the plantar fascia, and failure of the pain to improve. 13 Surgical procedures, such as plantar fascia release, are a last resort, and often lead to further complications such as a lowering of the arch and pain in the supero-lateral side of the foot due to compression of the cuboid bone 14 An ultrasound guided needle fasciotomy can be used as a minimally invasive surgical intervention for plantar fasciitis. For plantar fasciitis, the two most important things to consider in selecting the right sneaker are the construction and fit. To be effective in helping to reduce the pain and swelling in the heel and arch, the sneaker must be provide shock absorption in the heel, good support in the heel and along the arch, and promote proper foot position. The fit should not be snug, and should instead provide a roomy toe box to help reduce the chance of more foot complaints like bunions occurring. When laced up, you should feel no pressure points on any part of the foot.

Surgery is the most drastic option and should only be considered once all the others have been exhausted. Plantar Fasciitis surgery may carry risks that include rupture of the plantar fascia, infection, and/or nerve injury that results in numbness in the region. There are also additional complications that may emerge post-surgery. For instance, if the plantar fascia is released too much during the procedure, the arch of the foot might be reduced. In addition, some people still suffer from symptoms and pain related to Plantar Fasciitis after their surgery is performed. A sports medicine physician will be instrumental in determining the best course of treatment.

Night splints usually are designed to keep a person's ankle in a neutral position overnight. Most individuals naturally sleep with the feet plantar-flexed, a position that causes the plantar fascia to be in a foreshortened position. A night dorsiflexion splint allows passive stretching of the calf and the plantar fascia during sleep. Theoretically, it also allows any healing to take place while the plantar fascia is in an elongated position, thus creating less tension with the first step in the morning. A night splint can be molded from plaster or fiberglass casting material or may be a prefabricated, commercially produced plastic brace ( Figure 8 )

The Achilles tendon is a group of tissues that connect the heel bone to your calf muscles. When these are inflamed, you have Achilles tendonitis. The plantar fascia is also a group of tissues; but, this links the toes to your heel. Home remedies work well for both the conditions. The treatment of plantar fasciitis and Achilles tendonitis are almost the same. When ice packs and stretching exercises don't work for plantar fasciitis, it may be advisable for you to visit a podiatrist. A new therapy called ESWT or 'Extracorporeal Shock Wave Therapy' is recommended. The therapy may take about 4 months for complete cure.

I then begin to send healing energy to the affected area of pain, in this case the heel of your foot. I send the energy for about 20 minutes and then ask you for your pain level again. A bunion is a painful swelling that usually occurs at the head of one of the long bones (metatarsal bones) of the big or little toe, which extend from the arch of the foot and connect to the toes. The way shoes are laced can be important for preventing specific problems. Laces should always be loosened before putting shoes on. People with narrow feet should buy shoes with eyelets farther away from the tongue than people with wider feet. This makes for a tighter fit for narrower feet and looser for wider. If, after tying the shoe, less than an inch of tongue shows, then the shoes are probably too wide. Tightness should be adjusted both at the top of the shoe and at the bottom. Where high arches cause pain, eyelets should be skipped to relieve pressure.

Treatment for Calluses. Calluses protect the feet and often do not need to be treated, but if a callus causes pain, several treatments are effective. Sanding the callus with a pumice stone after bathing is very helpful. Soft cushions placed in the heel of the shoes or under the ball of the foot can help relieve pain. Specially fitted shoe inserts are also helpful.
